what would cause a serpentine belt to come off. It is not broke or cracked. came off while driving

More than likely you have a bad belt tensioner, and bad idler pulleys that the belt rides on. Should replace all idler pulleys and the belt tensioner.

Also check crank damper pulley if rubber Vibration damper is cracked or oily and soft Pulley can flex and throw belt off.
