Wheel bearings

I have replaced all 4 wheel bearings and even
replaced the front two thinking they were faulty.  A
couple months later it is vibrating really bad over
40km.  Could it be the axles or just an alignment?

"you " replaced them or some mechanic did ? So without checking pressure , condition or balance of the tires or any number of front end components they put all new bearings on without solving the problem , I think you need another mechanic.

If the vehicle ran OK for the first few months, I would suspect that you threw a tire weight and just need to have the tire's balance re-checked.
