I am guessing your engine has a lot of blow by due to wear. Check the PCV to make sure it is working properly. The mis-fire may be due to oil fouled spark plugs.

I would do a complete tune-up spark plugs spark plug wires new PVC valve all your filters fuel filter all these things and then see what happens are you getting any codes?
