My 2012 ram had the same issue. The misfire on #4 started at around 150K. Soon after I started finding coolant in my engine compartment which led to overheating. this was intermittent. it got worse and worse. I was getting coolant in the #4 hole. it would only leak when hot but when it did it was pressuring my coolant system and pushing it out the cap. the tick (If you don't have an exhaust leak is a lifter tick. common with the hemi of this era. the lifters develop flat spots and tick with the possibility of wearing your camshaft. I am in the process of swapping my engine with a newer one. Easier and close to the same cost of repairing it.

i'm having these issues right now . Cylinder 4 misfire swapped coil from 4 to 2, plugs from 4 to 6. Got cyl 6 misfire, Plugs had a whitish residue on them. Replaced plugs, cyl 4 misfire came back. Seems to be coolant leaking into 4 and causing misfire. Also has ticking, which they tell me is probably my cam and lifters. Now getting cyl 4 misfire and P2181, cooling system performance and overheating. Going to replace engine as the cam and lifter repair will cost too much compared to replacing engine
