Sounds like the gear selector switch or wiring is messed up. What does the scan tool say on the live data or any actual codes?

You don't see this very often. What you have is a transmission with seals that are worn out. Seals are leaking pressurized fluid out of their ranges and into adjacent ranges and countering pressure in them. All forward gears are neutralized and you're left with reverse only, and in your case, that's leaking into a forward gear. Your fifteen year old trans needs to be replaced.
