Wheel seal rubs on the spindle and the spindle lock nut loosens up. Does anyone know why this happens?

i replaced the rear wheel bearings on my 1992 town and country 2 wheel drive.

after i replaced the wheel bearings i noticed that the van will shake while driving. i lifted the van and noticed the the wheel was loose. i removed the tire and noticed that the nut was loose. why does the nut loosen up? i also noticed that the wheel seal rubs on the spindle and basically grinned the seal off. can anyone tell me if there should be a spacer there? or did something break?

Jimmy the wheel spindles either have a hole in them that a pin goes through, or you take a chisel and hit the corner of the bolt to distort it. out of curiosity, does the original bolt have a mark where it was struck previously? if so get a new bolt, they are designed to be used only once.
