Try cleaning the throttle body with a throttle body cleaner, also clean the PCV with the throttle body cleaner. I would also put some injector cleaner in the gas for a few weeks.

Could be IAC motor sticking, you could try cleaning the throttle body, the passages and the motor itself, if that doesn't work then I suggest you have the system scanned just too check the sensors to make sure they are working properly, if some of them are sending in correct signals the computer could crank up the idle thinking it is still a cold engine.
