My truck only starts when I take out and and put the starter relay back.

Every once in awhile my truck will have absolutely no charge, i am not able
to turn on any lights or do anything that requires power. I have tested the
battery multiple times and discovered that it is fully charged each time.
Usually I am able to get power back and get my truck running once i
remove the starter relay and put it back in. I am not sure why this happens
and I have no clue how to fix it.

Try putting in a new relay.

just like my last puzzle, this needs solid grounds...the battery has two 40 amp connections at the black terminal...one goes to the block or accessory mounts and the other goes to the chassis/fender...this one has some salt/crud and needs to be cleaned...start with a brand new battery cause it will have the duty cycle and won't go soft on ya~
