I just purchased a 2004 silverado with 158k 5.3 ltr engine is
there anything I need to worry about how many years miles
can I get out of this engine before something goes bad

Nothing out of the normal, its hard to say the parts that needed to be with the miles on her may have been done. Just make sure you always change the oil every 6,000 miles.

With proper maintenance it should run for a good long time.

Mine had 203,000 when it was wrecked - totaled. I replaced the water pump and the steering gearbox under the hood. That's all and it didn't use any oil between changes.
