Typically a certified used vehicle is checked by the service department of the dealer who is selling it. The vehicle must meet certain standards before it can be advertised as a certified used vehicle. For example, if you're looking at a Ford certified used vehicle at a Ford Dealer it has been checked by their service department, and meets Ford's standards for a certified used vehicle. Mileage, no accidents, and normal wear and tear specifications are met so the vehicle is advertised as a Ford Certified Used Vehicle. This is why it's usually a safer bet to buy from a dealer who sells new vehicles as well. You won't find older models with higher mileage and a jaded history of accidents, theft recoveries, and God knows what all else like you will at a small used car dealer or the auction.
