3rd gear only when RPM needle don't work

I have a 93 F-350 Diesel  Super Duty
automatic pickup truck. Sometimes the RPM
needle don't work and it only will take off in
3rd gear unless I manually shift it and then it
will take off in 2nd gear. Then other times
the RPM needle is working and it shifts
properly.
