Possibly warped rotor. But could be something more serious. Have it checked out by a certified brake technician.

okay, which year is this Mazda? sounds like collapsed brake hoses, to me~
Good catch operator_13. That very well could be it.
When I drive on the freeway and brake from 75 to slow down its a little hard to push on then it vibrates a little.
