99 ram 1500 starting issue

My v6 99 ram 1500 has an odd issue. i can
start it if i hold the accelerator down a bit,
and if i run it like this for a while it will stay
running in its own. if i try starting without
doing this, it will start and immediately die. i
have replaced the starter, replaced the TPS,
and cleaned the IAC. Short of a vacumn leak,
i cant figure out what the problem is(i really
dont want it to be a vacumn leak). Can
anyone help?

any trouble codes for iac. See trouble code below and possible causes. When you open the throttle, you bypass the iac. Did you clean throttle bore/throttle plate and small air passages?
