Possible switch or lose wire. Check the ground wires and make sure they are clean and connected. Then unhook the hot wire to the fan and see if it goes off. It may indicate a bad switch, or are other wires touching the switch or rubbed together and bypassi9ng the switch wire and putting a current to the fan?
