2000 Jeep Grand Cherokee V8 4.7, acts like its not shifting gears properly, when i come to a stop or put it in park there's a rough idle, rpms jump up down, and sometimes dies...?

Ive owned a 2000 Jeep Grand Cherokee limited for about 5 years now and
just started experiencing problems for a couple weeks now. First, let me list
some maintenance history and previous problems... jeep has 230,000
miles on it and Ive had several problems over the years with this vehicle
and my previously owned 1996 jeep grand cherokee, much of them were
the same. Ive already been through hell with both vehicles pertaining to the
stalling, loss of power steering, and not being able to fire back up most of
the time while driving. Ive replaced many crank and cam sensors, including
mopar parts, only for the problems to persist. Eventually a replaced
computer on my 2000 jgc, after many failed attempts of replacing sensors
among many other things, did the trick and its been running good for a
couple years now. Ive also had the engine overhauled, replaced the
transfer case, alternator, water pump, battery, belts, to name a few. My
current problem is different from the prior stalling that so many have been
talking about...for the last 2 weeks while im driving at low speed it acts as if
its not changing gears properly and it jerks sometimes or boggs down
when i accelerate. When im coming to a stop and idling or in park, the rpms
jump up and down between 500-1500 before usually dying. It always
cranks or turns over but sometimes doesnt fire back up. After having a
friend who is a mechanic experience the problem im having he thought it
sounded like it wasnt getting fuel so we changed the fuel filter but the
problem persists. Before I replace the fuel pump, and such does anyone
have any idea of what the solution might be?
