Looks like the belt tensioner on my 1998 ford zx2
is out of alinement. Would this be the reason my
belt came off. Or is that the way it looks

If it's questionable just replace it. If it can't keep adequate tension on the belt it will slip or walk off.

Everyone here is right. However, I do not recommend aftermarket belts for these cars esp dayco. I have had nothing but trouble with those. The grooves aren't nearly as deep as the ford oem belt.
