Low fuel pressure or supply, plugged exhaust system, cam or crank sensor erratic signals, rev limiter malfunction.

I would assume a check/service engine light would be on, no? Scan for the codes if it is.

Not really, I've had a a fair number of rigs that came in and no codes were present but did show erratic signals from either the crank or cam sensors, had a Chevy truck show both and still no code.
