I have the exact same '95 van as yours. It is most likely the fuel pump, but as Kelly says, you should check the cheaper, easier things first. I've changed my pump twice. The first one was second hand, and it lasted less than 2 years. The next, (new) one is still running. Avoid letting the van run low on fuel. Don't let it go below quarter tank. Dirt collects on the bottom. If you're doing this job yourself, you'll have to drop the gas tank down to remove pump. An empty tank is lighter and easier to work with. You should be able to find a YouTube video of somebody doing the whole job. For sure change the filter. Good luck.
