2003 Taurus Dohc 6cyl. Ive been having this concern about my rpms reving high now this only happens occasionally on takeoff. It feels as if the car Just halts and suddenly revs up to 3000-4000rpms

The car Will shift to normal usually after a
few seconds of rev up.  My concern is
getting rear ended.  Ive had transmission
fluid and filter change still does it

Sounds like a converter issue.. something is causing your converter to let go.. your pcm has a module and driver for it. Is there an engine light on? Possibly for a coolant temp sensor or tps sensor?
