Transmission Problem

I was heading back from work the other day about a 45min commute and just as i'm
getting off my exit i felt the car slip and loose power it felt like for a quick second and
looked at the rpms and it spiked 1,00 rpm when this happened and continued to shift
weird like this until i pulled over and turn the car off and turned it back on. This
happened again yesterday on my way back home from work but not when i was
heading to work, about 70mile round trip. This time it slipped only once when i was
getting off on my exit and acted fine after that.
The code that are showing are P0420 (Emission code) and P0456 (gas vapor code).
Has anyone every came across this yet?
