Have 2012 Grand Caravan with 6 cyl. At 48,000 miles, I had similar issue. Took it to dealer and they replaced the head under warranty. Said this will fix the issue. Car just passed 96,000, same issue. Took it to the dealer. They are going to replace a bunch of internal (lifters, rockers, etc) along with possibly the head again. Known issue. Normal 30 mile commute into work, 30 miles home. Highway speeds of 60-70 mph (Dallas, Tx). I would take it to the closest dealer.

Had to replace head/valve on left side under warranty (124k miles but they cover to 150k if 2011-2013 as there was a design flaw and initial fix in 2012 still fails). Only improved it so brought it back and they found a bad coil as well. Now with both fixed seems to be running good (60 miles so far) - only $159 total for fixes.
