Possibly a mode actuator going bad or actuator door.

Is it difficult to fix and how do you diagnose which it is?

Most times these are not an easy fix. Because they are buried deep behind the dash. The actuator is hooked to the shaft of the door. When it is located you will either find the actuator not operating properly or the door broke loose from the shaft.

So basically if I find the actuator and twisting the shat by hand moves the door it is the actuator

If the actuator is turning the shaft but the air does not change where it's coming out then it is probably the door. If actuator is turning but not moving the shaft the gears in actuator are probably bad/broken.

I have a 2008 with a similar problem except my floor settings blow out the panel vents. If i set it to the floor/panel combo setting i can get a little floor heat and only on the passenger side. What do u think this could be?
