Is this the throttle positioning sensor? Or more major transmission problem?

Van rpm run at 4,000 but vehicle strains to do 45-50 mph.  Gas mileage is very poor.

It could be a plugged catalytic converter or drive and overdrive might be out in the transmission. Start by checking the transmission fluid level. It should be running when you check it. the fluid should be red like cherry Kool-aid and not smell burnt. If it's low fill it to the proper level and pray that the transmission isn't junk. Dodge/Chrysler van transmissions are famously horrible. Here's hoping for a plugged converter!
