Radiator fan runs all the time when engine running

My 2000 T&C radiator fan keeps running after only a few minutes on the road.  It is also overheating a little from time to time.  Cools off when i slow down.

Have you had a coolant flush or belts replaced since owning the van? The fan will also run whenever the A/C is on & the compressor is engaged.
