My 1998 2500 chevy express van has milky oil in the radiator

My radiator has oil in it, I read that big truck or vans have oil coolant system i noticed
that from the oil filter there are two hoses going to the radiator do i need to replace the
radiator?

Hopefully you don't have a blown head gasket. The two hoses going to the radiator are for oil cooling but the cooling tube in the radiator may be leaking oil into the coolant. YES replace the radiator and have the cooling system flushed and use an external oil cooler. DO NOT drive the truck until this is done, you can severely damage the engine if you do.
