98 dodge ram

I have a 98 Dodge Ram that will not shift out of
first it has a 46re transmission it has the 318 in it
and it does not want to rev up and I have a rattle
that sounds like it is in the exhaust that it did not
have before

Sounds like 2 problems you're describing here. First, no revving the noise in your exhaust: the cat is prolly bad, n 2nd, won't shift outta first, fix the first, first. If the tranny is shot, that's lots more money.
