Why do my 91 crown vic jerks when driving and goes slow n jerk up hill?

i have changed egr valve, fuel filter. transmission filter, solinoid, and tps please help its
fustrating me,,,,,thank you

Maybe a shift sensor on the transmission not certain tho have you put it on a code reader?
