Had my sienna coolant drained and flushed. Since my coolant capacity is a
little over 12 Qts, i supplied 1 gal. toyota concentrate and 21/2 gals. of distilled
water. After the job was done he left 2 whole gals. od distilled water. The next
day i noticed the coolant reservoir nearly empty. Was this a botched flush?

1 gallon anti freeze and 2 1/2 gallons water is all wrong. You need a 50/50 mix. 12 quarts is 3 gallons. You should have installed 1 1/2 gallons anti freeze.
