Grand Am hard shift

My 2001 Grand Am has been idling
roughly, hesitating when I accelerate from
a stop, and shifting really harshly. It would
be driving fine, but if I were in stop-and-go
traffic for any length of time especially
when it's hot out, it would start doing all
that. I just replaced the TPS and the rough
idle and hesitation to accelerate stopped,
but after a little bit, it still shifts really
harshly and jolts the whole car. Any idea
what could be causing that?
