The 2003 Caravan is equipped with a power steering rack so you either have an internal problem within the rack or you blew out an outer tie rod end. Check the outer tie rods first obviously and make sure they are mechanically sound. It would be an easy fix if it's an outer (closest to the wheel). If the problem is inside the rack, it's garage time.

Sounds like a tie rod issue. But there are some other items that could cause this. Jack up vehicle and inspect steering components and ball joints.
