when do you know you need to replace the timing belt?

You will not get a warning. Replace it on schedule which for your car is every 105,000 miles. If there is an inspection window your mechanic can look at it and see if it is ready to fail.

Has it ever been replaced? 2003 is 14 years ago.. that's a long time.
