Is my clutch broken?

Hi I have a mk4 fiesta It worked fine
but a week ago it starte playing up
you put the car into gear and it
doesn't really react to the fact it's in
gear untill Uve got the revs to redline
it will then start to pick up and the
revs go back down it was doing it
every now and then and now it doing
it every time it goes in gear untill Uve
red lined it what is this??
