I have a Mercedes E 320 wagon which is using oil and my mechanic say the first piston is shot, due the nickel wearing insided the piston.

He suggest buying a used engine, however, before this began happening he changed the  valve cover gasket to the engine. I have own the car the entire time and I have used synthetic oil for at least 10 to 12 years,
Should I buy a remanufacture engine or a used, are have the cylinder fixed if possible?

Buy the replacement engine. Much cheaper than rebuilding an engine, especially as yours needs machining. (Then again, with Mercedes the prices are probably through the roof no matter which option you take.)

I agree were past the "age" of engine rebuilding its much ore cost effective to install a low mileage replacement , salvage engines are everywhere thanks to online search capability and are low cost!
