Hard to tell from where I sit, open the hood and check the condition of the hood and belt. How are the motor mounts? If they are worn, the front of the engine can sag and the fan starts hitting the fan shroud.

A squirrel or cat?

I had the same problem a year ago with my aztek. In my case it was a spark plug wire that would swing into the fan when I was driving.
