Why does my 2003 MAZDA6 act like it is going to die?

My 2003 Mazda 6 likes to act like it will die when
you first start it up. You have to warm it up before
even moving. The temperature gauge has to be
almost in the middle for the car not to run like crap.
When you first start it  it will act fine then all the
sudden rev high, then low, then get the hiccups,
then either die or act like it will die. Then after I put
gas in the car. Sitting at the pump it will act like it
will die at the gas station. Then when you are on the
road it runs perfectly fine. I just need help pin
pointing what the issues are.
