Lexus rx300 2003 with 148k miles Timing chain overdue? How critical and how much?

It does not have a chain, it has a belt. And the belts are called to be replaced by the manufacturer every 90,000 miles. Dealership is going to get 4.5 hours laborat 120 dollars an hour plus 60 dollars for the belt. Once inside the will be able to see cam seals, etc that may need replacing. Water pumps are common on these to leak too and the belt must be removed to change that.
