Clutch disc

I'm helping a friend replace his clutch on a
1995 Honda civic. It was making an awful
grinding noise. On inspection of the clutch
disc I noticed all the splines were gone. Is it
just wore out or does he have transmission
problems. I put a used transmission and a
new clutch set about three years ago
