How do you replace flywheel on 99 gmc
suburban

The transmission must be removed first.

It all depends on what a guy feels comfortable with, removing the transmission or the engine, if it's not a 4X4 then the tranny might be the easiest way to go, just remember to replace the transmissions front seal and the engines rear main seal while your there, it could save you a headache later on down the road.
