whats wrong with my car

my car all of a sudden wants to stall usually when I come to a red light or stop on a slight incline. When this happens it takes a few try,s to get it started again also it has these episodes where I will be driving and the car will start jerking and when it does this I also loose power and cant get it over sixty can you please help me

check for vacuum leaks. also, the jerking can be the lockup solenoid in the transmission. if you can locate it, unplug it and see if it changes.
